Congratulations on your ABQAURP HCQM Certification accomplishment!
Certification from the American Board of Quality Assurance & Utilization Review Physicians, Inc. represents a significant professional achievement, as well as provides the prestige of being a distinguished expert in the fields of Health Care Quality Management and Patient Safety.

As an active certified Diplomate, you gain access to the ABQAURP Certified Diplomate Logo. Upon approval of use, the Logo may be used as further confirmation of your commitment to “improving the overall health care provided to the consuming public.”

To request use of the ABQAURP Certified Diplomate Logo ("Logo"), please read the terms of this Logo License Contract ("Contract"), and indicate whether you accept all of the terms by clicking ACCEPT below. Click DECLINE if you do not agree to all the terms. Provide all of the required information on the following page. If ABQAURP accepts your application and agrees to enter into the Contract, this document will be a legally binding contract between you and ABQAURP. Once your application is accepted, ABQAURP will provide in an email, a link to access the Logo via ABQAURP's website. You may not use the Logo if you do not accept all of the terms in the Contract. Logo License Contract


Purpose:
ABQAURP wants to enhance the visibility of the organization and ABQAURP certified health care professionals (“Diplomates”) to the public. Allowing Diplomates to utilize the ABQAURP Certified Diplomate Logo (“Logo”) will provide Diplomates with an additional benefit to certification and will increase recognition of ABQAURP certification throughout the health care industry and to the consuming public.

Terms of Use
Use of the Logo is restricted to Diplomates only. The Diplomate is responsible for the manner in which the Logo is used in association with his or her name. Consistent with the Language of Use outlined below, the Logo can be used only when accompanied by the appropriate disclosure of certification.

ABQAURP certification is an attribute of the Diplomate and not of his or her practice or employment organization. When using the Logo, it must be clear to a reasonable person the certification applies to an individual physician, nurse or other health care professional, and the identity of that person must be clearly specified. The Logo may not be disclosed in any manner in which a reasonable person is led to believe a practice or organization is certified by ABQAURP.

Diplomates may not use the Logo in a manner violating applicable ethical rules of the state or locality having jurisdiction over the Diplomate. It is the responsibility of the Diplomate to determine the nature and scope of such ethical restrictions in the relevant jurisdiction.

Subject to the requirements of this contract and the restrictions under applicable ethical rules, Diplomates may display the Logo in any communication. Appropriate uses would include the use of the Logo in newspaper advertisements, yellow page listings, letterhead, business cards, pamphlets, brochures, television advertisements, and Internet displays.

Diplomates are permitted to use the Logo only in accordance with this Contract and only as long as they remain actively certified by ABQAURP.

ABQAURP may withdraw a Diplomate’s right to use the Logo for any reason upon written notice to the Diplomate. If such notice is provided, the Diplomate must cease use of the Logo within 30 days of the date of the notice, and may no longer hold himself/herself out as a Diplomate. After the 30-day period, the Diplomate may not use any printed material, Internet display, or other communication containing the Logo. Advertisements placed prior to receipt of the notice need not be withdrawn, but no such advertisement may be renewed after receipt of the notice. (e.g. A Diplomate who ceases to be in good standing need not take any action with respect to a yellow page advertisement containing the Logo and was placed prior to receipt of the notice. However, the Logo must be removed from the advertisement if it is renewed.) The Diplomate is responsible for any costs associated with ceasing use of the Logo.

In order to preserve the accuracy of the Logo and the quality of its presentation, only a Logo slick or electronic version provided by ABQAURP may be used for reproduction purposes. The Logo may be enlarged or reduced in size, but any other alteration of the Logo is strictly prohibited.

Written Disclosure
Except as restricted by applicable law, a Diplomate holding a current certificate from the American Board of Quality Assurance and Utilization Review Physicians, Inc. shall use the following language to disclose his/her certification in written communications:
