HOME STUDY COURSES
ABQAURP continuously strives to offer today's health care professionals convenient ways to increase and maintain their continuing education. Offering home study versions of our conferences provides access to our content with flexibility, allowing them to customize their education style and timeline. Additionally, Diplomates may utilize these courses to maintain their Diplomate status with the Board.

We currently offer two types of home study experiences, audio CDs and/or multimedia CD-ROMs/DVDs. Availability of media varies from course to course. All courses include a course syllabus and a post test. To meet the requirement for recertification, the post test must be returned six (6) weeks prior to the recertification deadline. Scoring results must be 70% or above to maintain your board certification.

Multimedia CD-ROMs/DVDs combine audio, video and presentation slides to reinforce the content being discussed. In addition to being a richer experience than standard audio CDs, the self-contained nature of CD-ROMs/DVDs means no downloading or waiting on your Internet connection like traditional online courses.

As we transition from volume based to value based care, health care providers and other stakeholders are struggling to identify key drivers to address the challenges around the redesign of health care delivery and payment reforms. What will be the role of integrated care management, ACO’s, and transitions of care models and how can we efficiently integrate all transactions of care across the care continuum?

Discover what the practitioner needs to not only survive, but to thrive in the evolution of health care. At the conclusion of the conference, participants should be able to:
  • Explain the relationship between hospital-based and community-based approaches to preventing readmissions.
  • Evaluate: Readmission or Post-Hospital Syndrome….what do you think?
  • Determine how value is rewarded through care redesign and improving transitions of care.
  • Identify and manage health care cost drivers through a well-designed care management program.
  • Decide if your coding, documentation, and payments are congruent?
  • Recognize community-based programs successfully employed to manage care transitions.
  • Discuss what you need to be a sought-after provider by EVERY payer.
“As Director of Specialty Programs at a post-acute infusion provider, I was searching for a forum to understand how changes in healthcare were affecting the acute care facilities so our programs could complement their strategies. The speakers at the Tampa conference were true thought leaders and provided a clear picture of where health care is going, what is being assessed and planned for and how the new ‘patient as a consumer of healthcare’ will impact those that do not accept change. Between this event and the National Transition of Care Coalition meeting in December I was able to strengthen our home infusion transition of care and patient outcome monitoring program to embrace the change and help our referral sources. I strongly recommend attendance and I am thankful to work for a company that is supporting my involvement.”
-Terri Street, RN, BSN

“Nearly one in five Medicare patients is readmitted within a month, which the government considers a prime symptom of an overly expensive and uncoordinated system.”

It is clear that CMS, and intuitively other payers of health care, are aggressively looking at every opportunity to improve efficiencies and reduce waste in all transactions of care across the health care continuum. To provide participants an understanding of, and tools needed to work successfully with, the rules for Medicare’s Hospital Readmission Reduction Program, ABQAURP presents an intensive one day event to address this tumultuous time in health care.

Whip your knowledge and skills into shape!

At the conclusion of the conference, participants should be able to:
  • Provide an overview and update of national strategies addressing transitions of care and reduction of readmissions
  • Discuss specific roles physicians and their extenders can play to reduce inappropriate readmissions
  • Discover Transitions of Care opportunities to reduce readmissions in high risk patients
  • Explore initiatives that have been successfully employed to reduce readmissions in hospitals

The health care system is evolving into one that will reduce costs by incenting physicians to provide timely, evidence-based, coordinated care; reimbursing them on the basis of efficiency, quality, and patient outcomes.

Small practices and groups are struggling to adapt to economic credentialing, provider profiling, and financial incentives with unclear boundaries. The specialist is facing being cut by the team; without knowing how the statistics are calculated. The patient and their family must play an active role. The payers must decide what strategy benefits everyone; do clinical outcomes align with performance measures at lower costs?

A team-based approach to care needs coordination among all those involved with the patient (i.e. primary care physician, hospitalist, caregiver, specialist, and case manager) to ensure safe, timely, and smooth transitions throughout the health care continuum.

At the conclusion of the conference, participants should be able to:
  • Discuss opportunities and challenges of the Medicare Shared Savings Plan
  • Recognize the concept of Medical Home
  • Evaluate CMS’ Interim Final Rule for Fraud and Abuse Waiver, i.e. waiver of Stark Laws
  • Discuss the application of Quality Measures in practice
  • Identify how quality measures and financial performance work together
  • Describe how the Guided Care health model operates in practice
  • Analyze the critical infrastructure factors for ACO success – health IT, care management, community workers, etc.
  • Define the continuum of care from a provision of services perspective of an ACO
  • Discuss real-life lessons learned from current roll-outs of ACO-type organizations
This course is intended for Physicians of all specialties, Nurses, and Other Health Care Professionals with appropriate CME credit and nursing contact hours offered for each profession.
