1 CEU Credits The Institute of Medicine defined patient safety as: Freedom from accidental injury in its landmark publication, To Err Is Human (1999). When it was published, estimated deaths from medical errors in US hospitals was 98,000 per year and, while many successful improvement initiatives have been implemented since then, questions still abound in the field of patient safety. Learn about the history of safety initiatives, the culture of patient safety, and more in this online course. The American Board of Quality Assurance and Utilization Review Physicians, Inc. (ABQAURP), is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.
