NTOCC Call to Action

This year the National Transitions of Care Coalition (NTOCC) convened a special task force to review and provide recommendation/revision enhancements to the NTOCC Care Transition Bundle: 7 Essential Intervention Categories. The Care Transitions Bundle was first developed and launched in 2011 to support individuals and organizations with essential interventions for improving transitions of care for providers, patients, and their identified family caregivers.

Providers, patients, and caregivers are invited to review the suggested revisions and provide comments or thoughts about the proposed changes. There are seven (7) individual sections to review and you do not have to review all seven, but it is encouraged. The public comment period is open for 30 days, ending on December 31, 2021.

How to use the Now Comment platform:

In order to include your comments on the proposed revisions to 2011 NTOCC Care Transitions Bundle 7 Essential Elements document, you must set up a new account, which will only take about 60 seconds.

1. Click this link to go to NowComment.com and set up your account so it will be attributed to you. Anonymous comments cannot be considered.

2. Click the big orange "Sign Up (free!)" button on the Now Comment homepage, fill in email, name, and credentials.

Once you click the orange “Sign up” button at the bottom of the page you will receive an activation link via email from Now Comment. (If the email doesn't arrive within a few minutes, please check your spam filter and/or use the “back arrow” in your browser to check for a typo in your email address.) Click the activation link in the email (or copy and paste in your browser’s URL window). This will confirm your email address and let you do the last step of setting up a password. You are ready to start your review and comment on the documents.

3. Click here to access all 7 of the the care transition bundle documents: https://nowcomment.com/blogs/296311 to see existing comments, reply, or start a new conversation.

NTOCC looks forward to your comments on the proposed revisions to the Care Transition Bundle: 7 Essential Intervention Categories. If you have any questions, please contact Val Emmons at (501) 712-8686 or valemmons@gmail.com
