Exam Fees: Professional Member: $645.00 - Non-Member: $745.00

Registration Options: You can add additional Sub-Specialty certification(s) to your registration if you intend to apply for any (or all) additional sub-specialty(ies). International Candidates should add the international application surcharge, if testing outside of the US.

Non-Members: To apply for membership first to receive discounted member pricing: Click Create a new account and apply for Professional Membership. Then come back to add the exam to your cart for updated pricing.

Please note: To return forms, scan and send via email to: certification@abqaurp.org. If you do not have access to scan, there are scan apps available for mobile devices. Email will ensure immediate access. For security and quality assurance purposes, please do not mail HCQM Exam Applications. Thank you!

Please note: The application fee is non-refundable and must be paid online before application will be processed. The exam registration fee must be paid online in conjunction with the application fee and is non-refundable, unless the candidate is determined ineligible to sit for the exam by ABQAURP.
