  • Recognize how innovative care models, such as Hospital at Home, can be implemented to improve care transitions and alleviate social determinants of health barriers.
  • Describe strategies to avoid medication error and manage patient pharmacologic needs to improve patient outcomes and provider burden.
  • Prioritize the role of the physician advisor in managing resources to improve quality and reduce inefficiency throughout the health care system.
  • Develop leadership skills to all the interprofessional team to operate with clarity, resilience, and purpose to foster psychological safety, reduce errors, and elevate care.
  • Employ community-rooted health programs to meet the needs of an aging population to improve access, reduce readmissions, and relieve health systems through policy and interprofessional collaboration.

PLEASE NOTE:
 Course materials and name badges will be distributed at registration. Attendees must complete the online CME/CE Verification to be eligible for credit.

CANCELLATIONS: All cancellations and transfers are subject to a $100 processing fee. A full refund, less the $100 processing fee, will be given if you cancel in writing by March 13, 2025. Thereafter, credits on account may be approved for extenuating circumstances. If you do not cancel and do not attend, you are still responsible for payment. ABQAURP reserves the right to cancel the conference and return all registration course fees. ABQAURP may reschedule programs or close registration when programs are sold out. For these reasons, registrants are advised against purchasing non-refundable airline tickets.

RECORDING: Attendees should note that sessions are audio and/or video recorded and may be published in various media, including print, audio, and video formats without further notice. By attending, attendees acknowledge these activities and agree to allow their image to be used by ABQAURP in association publications, on ABQAURP’s website, and in marketing and promotional materials.
