NTOCC TOC Pharmacy Outcomes Survey Results

In 2023, the National Transitions of Care Coalition’s (NTOCC) Transitions of Care (TOC) Pharmacy Advisory Committee and its Sub-Committee on Value-Based Care & Outcomes created and sent out the NTOCC Transitions of Care Pharmacy Outcomes Survey to the NTOCC Community and their social media followers, including some ABQAURP members. 

The committee will use participant responses (see infographic below) in an effort to understand where there are opportunities to improve transitions and care coordination and integrate pharmacists within clinical care teams. NTOCC acknowledges that while no one discipline is wholly responsible for care transitions and coordination, they do advocate that a larger involvement by pharmacists beyond medication reconciliation will help realize an improved standard of care, including medication management services and overall quality of care as well as patient satisfaction and safety.

 

In support of this initiative, and to address issues and concerns that providers, patients, and their family caregivers continue to face with transitions of care, NTOCC offers a Lunch & Learn webinar series at no cost to the public and the NTOCC community.  Sessions are recorded and made available for a limited time, most offering CME/CE credit.
