Working With the TOC Pharmacists: Beyond Medication Reconciliation
Sara Panella, PharmD, BCPS
Manager of Pharmacy Ambulatory Care, Population Health and Transitions of Care
Baptist Health South Florida
Pharmacists, as members of the interdisciplinary care team, have a crucial role and impact in improving transitions of care (TOC) for patients, their identified family caregivers, and providers within the health care continuum. What we find today is their role is not always acknowledged within the collaborative care team, and we often exclude an important discipline that can support not only medication management services but interventions toward improving transitions and care coordination at all levels of care/service in our health care system. This course addresses the gaps and barriers that hinder TOC pharmacists and offer principles and examples of innovative collaboration that support pharmacists as an integral element of quality care transitions between patients, their identified family caregivers, and providers.
At the conclusion of this activity, attendees should be able to:
Complimentary for ABQAURP Diplomates and Professional Members
- Identify the gaps and barriers in transitions of care (TOC) and medication management services
- Discuss the role and function of TOC pharmacists and their potential impact to improve care coordination
- Analyze the Transitions of Care Pharmacists 10 Principles and how to incorporate this process into daily practice at the acute and post-acute levels of care
August 31, 2023 through September 30, 2023*

Innovations in Population Health
Amy Berk, MSN, RN
Director, Population Health
Innovations in population health begin with the end in mind. Improved clinical, operational, and financial outcomes are the underpinnings of a successful population health framework. Achievement of these outcomes is dependent on a combination of people, process, and technology; and it is technology that is the enabler to this plight. Technology that is led with innovation, and innovation that optimizes the population health framework - a framework that is grounded in data and AI. It is this robust data environment that provides enriched data activations expounding across insights, interventions, and outcomes.
Cloud technology brings forth capabilities that enable better experiences, better insights and better care. Enhancing patient engagement, empowering health team collaboration, enhancing the clinician experience, and improving clinical and operational insights are in alignment with the Quintuple Aim and are tangential to population health management. Cloud technology enables an integrated, scalable, and exchangeable data environment that promotes data quality and fluidity across the population health ecosystem including both providers and payers, thereby surfacing the right information at the right time to the right persona. It is this connectedness that facilitates more informed decision making, enhanced care collaboration, and a seamless experience amongst both providers and patients resulting in improved whole health outcomes, improved patient and provider satisfaction, and a reduction of costs across administrative functions and clinical operations.
At its core, Cloud is an integrated, scalable, and agile platform that serves as a secure data platform with an overlay of innovative solutions. Innovative solutions such as AI/Open AI is prime to transform population health initiatives by providing predictive, insightful, intelligent, and automated workflows across essential workflows to enhance the patient & provider experience, indefinitely change the way providers work, and the ways in which patients engage with their healthcare.
A myriad of applicable use cases is illuminated to accelerate population health initiatives spanning innovative care delivery models, e.g., digital health and virtual health, enhanced care coordination, precision medicine & genomics, whole person care and social determinants of health, health equity advancements, value-based care initiatives, and quality & administrative processes that support enhanced clinical, operational, financial outcomes that population health elicits.
As the dynamics and culture of healthcare change due to regulatory, equity, cost pressures, SDoH, environment, and changing population demographics, it is certain that the demand for population health will increase. As a result, population health will have a greater dependency on technology and innovation to accelerate initiatives that are prudent to population health’s key value proposition - more personalized care to improve health outcomes, advance health equity of the population, and reduce total cost(s) of care. This proposition is as important as ever and it is now time for providers and payers to take advantage of the benefits innovation can deliver to improve health outcomes, advance health equity, improve the clinician and patient experience, reduce total cost of care, and most importantly advance population health towards its future in healthcare.
