Pharmacists, as members of the interdisciplinary care team, have a crucial role and impact in improving transitions of care (TOC) for patients, their identified family caregivers, and providers within the health care continuum. What we find today is their role is not always acknowledged within the collaborative care team, and we often exclude an important discipline that can support not only medication management services but interventions toward improving transitions and care coordination at all levels of care/service in our health care system. This course addresses the gaps and barriers that hinder TOC pharmacists and offer principles and examples of innovative collaboration that support pharmacists as an integral element of quality care transitions between patients, their identified family caregivers, and providers.

At the conclusion of this activity, attendees should be able to:
  • Identify the gaps and barriers in transitions of care (TOC) and medication management services
  • Discuss the role and function of TOC pharmacists and their potential impact to improve care coordination
  • Analyze the Transitions of Care Pharmacists 10 Principles and how to incorporate this process into daily practice at the acute and post-acute levels of care
